Registered Nurse (RN) or Licensed Practical Nurse (LPN)

JOB SUMMARY:

We are seeking a dedicated and experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team. The ideal candidate will have at least six months to one year of experience as a nurse and a passion for making a difference in the lives of our clients.

KEY RESPONSIBILITIES:

• Provide comprehensive nursing care to clients in their homes, including assessments, care planning, and implementing nursing interventions.
• Administer medications and treatments as prescribed by physician/provider.
• Monitor and document client health status, including vital signs and progress.
• Educate clients and their families on health management and disease prevention.
• Collaborate with a multidisciplinary team to ensure coordinated and effective care.
• Respond to medical emergencies and provide acute care as needed.
• Maintain accurate and timely documentation of all care provided.

QUALIFICATIONS:

• Current RN or LPN license in the state of Indiana
• Minimum of 6 months to 1 year of experience in home health care, long-term care, nursing home, hospital, or DON
• Strong clinical skills and knowledge of home health care protocols
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• Compassionate, patient-focused approach to care
• Valid driver’s license and reliable transportation
